A Journey of Reablement and Hope
Background
Jon had complex and compound needs and comorbidities. After a lengthy period of detention Jon was referred to our West Sussex Outreach Service for support with transitioning to independent living in the community.
Support and Interventions
Working collaboratively with key partners to develop a tailored support plan focused on Jon’s goals. Key steps included:
- Staying Well Plan: Implementing an alert system to help Jon recognise and manage early signs of mood changes.
- Community Engagement: Supporting Jon in securing voluntary employment and in independent use of public transport to achieve this.
- Independent Living: Assisting Jon in learning budgeting and financial management, enabling him to independently handle bills and mobile banking.
Outcomes
Jon achieved remarkable milestones through the service including:
- Regular volunteering and steps toward paid employment.
- Financial independence and self-management of his home.
- Successful discharge from our service, having built a stable and independent life.
