What information do we process?
We may ask for or hold confidential and personal information about you, to enable us in providing you with high quality support services. This information may be provided directly by you, or via a healthcare professional, or in referral information.
The records may include:
- name, address, date of birth and next of kin
- records of contact we have had with you
- information from people involved in your care and treatment; such as health professionals and relatives
- sensitive information including sexuality, race, religion, disability, allergies, or health conditions
This information may be used:
- to help us understand your wants and needs, helping us to make decisions about your support
- to work effectively with other organisations who may be involved in your treatment plan
- to ensure services provided are safe
- to assist in the review of support planning
- to evidence the spending of public funds to commissioning bodies
This is positive to you because:
- sharing information provides you with the best quality, and most appropriate services
- it allows us to work in partnership with other professionals; ensuring that they have access to information and can provide you with the best possible care, without delay
How is my information kept safe?
All personal and confidential information shared with the organisation, is shared only on a need to know basis and access is restricted. We follow the Common Law Duty of Confidentiality and the Data Protection Act 2018. Information provided in confidence will only be used for the purposes to which you consent to, unless there are other circumstances covered by the law.
Our organisational policies describe the strict actions which employees must take to protect information and maintain confidentiality.
Who is information shared with?
To provide you with the best possible service, there may be occasions where we will need to share information about you with others, from whom you are also receiving care services. This may include NHS or health and social care organisations. This will be for a specific reason and is governed by specific laws.
We will never disclose any health information to third parties without your explicit consent, unless there are exceptional circumstances, such as when the health or safety of others is at risk or where the law requires the disclosure of information.
Your rights to information

How we use cookies

Data breaches
