Specialist Community-Based Support

Venture People provides specialist community support for people with complex mental health needs and compound needs. These services are designed to support timely hospital discharge, reduce readmission, prevent inpatient admission and provide intensive one-to-one support where a person needs a higher level of input.

Who Specialist Community Support Is For

Specialist Community Support may be suitable for adults who need intensive, flexible or time-limited support in the community.

This can include people leaving hospital, people at risk of admission, people using crisis or urgent care services, and people with complex or compound needs who require structured one-to-one support.

Support can include help with daily routines, budgeting, coping strategies, community reintegration, tenancy support, wellbeing, confidence, social connection and skills for independence.

Venture People offers a range of specialist community-based services tailored for people with diverse and complex needs, including mental health needs, learning disabilities, neurodiversity and associated complex needs.

These services are recovery and reablement focused, person-centred, psychologically informed and trauma-informed.

We are proud to offer innovative, outcomes-based specialist community support for people who need it most. This includes people who need support to facilitate discharge from hospital, avoid hospital admission or stabilise safely in the community.

Support is flexible and shaped around each person’s needs, goals and circumstances. No one service looks exactly the same.

Specialist Community Support may be suitable for people with mental health needs and additional needs, including dual diagnosis, learning disabilities, autism, neurodiversity, forensic history, complex emotional needs or trauma backgrounds.

DORS

Discharge, Outreach and Recovery Support.

Overview: Designed to support timely discharge from hospital and reduce readmission rates, DORS provides intensive, short-term community support. Referrals are responded to within one working day.

Support: Up to 200 hours of one-to-one support over six weeks, assisting with budgeting, coping strategies, social reintegration and tenancy support.

Outcomes: In its first year, DORS supported 166 people and helped facilitate significant cost savings by reducing hospital readmissions. The service has received a 100% positive outcome rate based on stakeholder satisfaction.

Rapid Discharge Service

Overview: This service supports fast-tracked discharges from urgent care and crisis teams. It provides intensive, short-term community support to help prevent inpatient admissions.

Support: Up to 100 hours of one-to-one support per week for the initial 12 weeks, focused on building confidence, daily living skills and connecting with community resources.

Outcomes: In its first six months, Rapid Discharge helped avoid hospital admissions for 36 clients, achieving a significant cost saving for the NHS. The service has received a 100% positive outcome rate based on stakeholder satisfaction.

Venture Onwards Service

Overview: A high-intensity single-person service for individuals with complex and compound needs who require daily, one-to-one support as a positive alternative to supported living.

Support: Up to 24 hours of individualised support, focusing on skill development, positive social interactions and daily routines.

Outcomes: 95% of Onwards clients avoid readmission to hospital, with 40% progressing to lower support or no support.

Specialist Community Support can include core and one-to-one support shaped around the person’s needs, strengths and goals. This may include:

• learning, employment, creative and leisure pursuits
• accessing and using the wider local community
• inclusion and social connection
• managing wellbeing
• developing effective coping mechanisms
• activities of daily living
• finance management and weekly budgeting
• skills for independence
