Community-Based Support
Venture People provides flexible community-based support for adults with mental health needs, learning disabilities, neurodiversity and associated complex needs.
Our community-based support helps people build confidence, strengthen daily routines, improve wellbeing and access the wider local community. Support is person-centred, recovery-focused and shaped around each person’s needs, goals and circumstances.
Community-Based Support can work alongside accommodation-based services or as a separate outreach service for people who need support in their own home or local community.
Who Community-Based Support Is For
Community-Based Support may be suitable for adults who would benefit from flexible, practical and emotional support in the community.
This can include people who need help with daily routines, social connection, wellbeing, independent living skills, confidence, budgeting, meaningful activity or accessing local services.
Support can be adapted over time as someone’s needs change, helping people move towards greater independence at a pace that works for them.
How We Support People
Support is built around the individual and the outcomes they want to achieve.
This may include support with:
- building daily routines
- managing wellbeing
- developing coping strategies
- accessing the local community
- building social networks
- meaningful daytime activities
- learning, employment, creative or leisure pursuits
- activities of daily living
- finance management and weekly budgeting
- maintaining physical and mental wellbeing
- developing skills for independence
- working towards personal goals
Recovery and Reablement
Our community-based services are recovery and reablement focused.
This means we work with people to build on their strengths, develop practical skills and increase confidence in everyday life. Support is designed to help people move forward, maintain progress and take greater control over their own routines, choices and goals.
Person-Centred Support
No one person’s support looks exactly the same.
Each person is supported to create a plan that reflects their needs, strengths, aspirations and circumstances. This plan can change over time as goals are achieved, new needs arise or the person becomes more independent.
Community Inclusion
Being connected to the local community can play an important role in recovery, wellbeing and independence.
Community-Based Support can help people access local services, attend appointments, take part in activities, reconnect with interests, build confidence in public spaces and develop stronger social networks.
Building Independence
Support can focus on the everyday skills that help people live more independently.
This may include support with planning routines, managing appointments, budgeting, preparing for education or work, accessing transport, maintaining wellbeing and developing confidence in daily life.
Measuring Progress
Venture People uses outcome-focused tools to help people identify goals, track progress and recognise achievements over time.
The Outcome Star can support people to reflect on areas such as health, housing, independence, relationships and meaningful activity.
ReQoL can also be used to measure quality of life for people with mental health conditions, including wellbeing, relationships, community engagement, empowerment and independence.
