Outcomes and Impact

Measuring progress, improving lives and showing the difference support makes.

Outcomes and Impact

Venture People is focused on helping people build confidence, independence, wellbeing and greater control over their lives.

Across our services, we work with each person to understand their goals, plan the right support and measure progress over time. This helps us evidence the impact of our work while keeping each individual at the centre of their own recovery or reablement journey.

Our outcomes are shaped by the people we support, their families, referrers, commissioners, partners and staff teams.

Measuring Progress

We use outcome-focused tools to help people identify goals, reflect on their current situation and recognise achievements.

This gives each person and their support team a clear way to track progress, understand what is working and agree the next steps.

Progress may look different for each person. For some, it may mean maintaining a tenancy. For others, it may mean reconnecting with the community, building confidence, improving wellbeing, developing daily living skills or moving towards less intensive support.

Outcome Star

The Outcome Star is used across our services to support people to reflect on their needs, goals and progress.

It helps people look at key areas of life, including health, housing, independence, relationships and meaningful activity.

By using the Outcome Star, we can support people to identify what matters to them, tailor support plans around their aspirations and measure distance travelled over time.

It also helps people recognise achievements and agree realistic next steps in their journey.

ReQoL

ReQoL is used to measure and improve quality of life for people with mental health conditions.

It focuses on wellbeing, meaningful activity, personal relationships, community engagement, empowerment and independence.

Using ReQoL helps us keep services recovery-focused and gives people a clear way to reflect on changes in their quality of life over time.

Outcomes We Aim to Support

Our services are designed to support meaningful and measurable outcomes.

These may include:

  • improved wellbeing
  • increased access to the local community
  • stronger daily living skills
  • greater independence
  • increased access to education, training and employment opportunities
  • sustained and successful tenancies
  • improved social networks
  • reduced reliance on statutory services
  • improved confidence

Independence and Daily Living

Many people we support are working towards greater independence in everyday life.

Support may include building daily routines, managing activities of daily living, developing budgeting skills, maintaining physical and mental wellbeing, accessing community resources and working towards personal goals.

The aim is to help people build confidence, skills and stability at a pace that works for them.

Community Connection

Community connection is an important part of recovery, wellbeing and independence.

Venture People supports people to access and use the wider local community, build social networks and take part in meaningful daytime activities.

This may include learning, employment, creative interests, leisure activities, volunteering or reconnecting with services and support networks.

Tenancy Sustainment

For people using supported accommodation, a key outcome is sustaining a safe and successful tenancy.

Support can help people build the skills needed to manage their home, maintain routines, manage finances, access local services and work towards greater independence.

Where appropriate, support can also help people move towards less intensive support or more independent living.

Reducing Reliance on Statutory and Emergency Services

Venture People’s support can help people develop safer and more constructive ways to seek help when they need it.

Through responsive support, out-of-hours support, community-based services and specialist pathways, we aim to reduce unnecessary reliance on statutory and emergency services.

This helps people feel more supported while also easing pressure on wider health and social care systems.

Specialist Community Support Outcomes

Specialist Community Support can help people move safely from hospital, avoid admission, reduce readmission risk and stabilise in the community.

Support may include intensive one-to-one input, daily routines, coping strategies, tenancy support, budgeting, social reintegration and confidence building.

These services are designed to provide focused, time-limited support when people need a higher level of input.

Person-Centred Support Plans

Each person’s needs are unique and may change over time.

Venture People works with each individual to create a support plan that reflects their circumstances, strengths, risks, goals and desired outcomes.

Support plans are reviewed so that progress can be tracked and support can adapt as the person’s needs change.

Working With Families, Referrers and Partners

Good outcomes are supported by clear communication and collaborative working.

Where appropriate, we work with families, carers, referrers, commissioners, NHS teams, local authorities, housing partners and other professionals to support consistent, joined-up care.

This helps each person receive support that is coordinated, responsive and focused on what matters to them.

Success Stories

Success can look different for every person.

For some people, success may mean feeling more confident, attending appointments, managing a tenancy or reconnecting with family. For others, it may mean joining a local activity, starting education or work, reducing crisis contact or moving to a lower level of support.

Our Success Stories share examples of progress, recovery, reablement and the positive impact of person-centred support.

Continuous Improvement

Venture People uses feedback from the people we support, families, staff, referrers and partners to improve services.

We value feedback because it helps us understand what is working, where support can be strengthened and how services can continue to respond to changing needs.

This commitment to learning helps us maintain high-quality, responsive and outcome-focused support.
