We are proud to offer a range of innovative outcomes based specialist community based solutions for individuals who need it the most. This includes those who need support to facilitate discharge from hospital, or avoid hospital admission.

DORS (Discharge, Outreach and Recovery Support)

Overview: Designed to support timely discharge from hospitals and reduce readmission rates, DORS provides intensive, short-term community support. Referrals are responded to within one working day.

Support: Up to 200 hours of 1:1 support over six weeks, assisting with budgeting, coping strategies, social reintegration and tenancy support.

Outcomes: In its first year DORS supported 166 people and has facilitated significant cost savings by reducing hospital readmissions. The service has received a 100% positive outcome rate based on stakeholder satisfaction.

Rapid Discharge Service

Overview: This service supports fast-tracked discharges from urgent care and crisis teams. It provides intensive, short term community support to prevent inpatient admissions.

Support: Up to 100 hours of 1:1 support per week for the initial 12 weeks, focused on building confidence, daily living skills, and connecting with community resources.

Outcomes: In its first six months, Rapid Discharge helped avoid hospital admissions for 36 clients, achieving a significant cost saving for the NHS. The service has received a 100% positive outcome rate based on stakeholder satisfaction.

Venture Onwards Service

Overview: A high-intensity single person service for individuals with complex and compound needs who require daily, one-to-one support.

Support: Up to 24 hours of individualised support, focusing on skill development, positive social interactions, and daily routines.

Outcomes: 95% of Onwards clients avoid readmission to the hospital, with 40% progressing to lower, or no support.
